Employer information

  • General Compliance Certificate: Employers will require this certificate to set up contributions into Simple Choice Super.

Forms for Individuals

  • Application Form: Complete this form to become a member of Simple Choice Super. Alternatively, you can download the printable Application Form.
  • General Compliance Certificate: Employers will require this certificate to set up contributions into Simple Choice Super.
  • Rollover Form: Complete this form if you are already a member of Simple Choice Super, and would like to rollover some money from another super fund into your Simple Choice Super account.
  • Personal Contribution Form: Complete this form if you would like to make a personal contribution of money into your Simple Choice Super account. If you are making a self-employed concessional contribution (before-tax), you must also complete the Notice of intent to claim or vary a deduction for personal super contributions form and send it to us with your Personal Contribution Form.
  • Life Insurance Application Form: Complete this form if you would like to access death, total permanent disablement, or income protection insurance through your Simple Choice Super account. Please contact us on (02) 8556 7576 or at [email protected] for this form.
  • Life Insurance Transfer Form: Complete this form if you would like to transfer your current insurance coverage from another fund to your Simple Choice Super account. Please contact us on (02) 8556 7576 or at [email protected] for this form.
  • Non-Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Simple Choice Super account in the case of your death.
  • Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Simple Choice Super account in the case of your death.
  • Withdrawal Form: Complete this form if you would like to apply to withdraw all or some of your super money.
  • Early Release Due To Financial Hardship Form: To access some or all of your super due to severe financial hardship, please read this information carefully.
  • Early Release Due to Permanent Incapacity: Complete this form if you would like to access all or some of your super money as you are unable to work due to a medical condition.
  • Adviser Services Form: Complete this form if you have a financial adviser and you would like them to have access to your account information. You can also complete this form if you would like an Adviser Service Fee paid from your super account to your financial adviser.
  • Change of Details Form: Complete this form to update your personal details with Simple Choice Super.
  • Contribution Splitting Form: Complete this form to split your superannuation contributions.
